Last week, we headed to\u00a0Glasgow for the\u00a0BSA Medical Sociology Annual Conference<\/a>\u00a0to share\u00a0a sneak preview of\u00a0our\u00a0findings\u00a0from\u00a0This Sickle Cell Life: voices and experiences of young people with sickle cell<\/em>.<\/p>\n

Sickle cell disease is\u00a0a genetic blood disorder\u00a0disproportionately found<\/a>\u00a0in\u00a0minority ethnic communities in Britain.\u00a0It is a chronic debilitating condition that\u00a0both\u00a0causes cumulative damage to multiple organ systems,\u00a0and\u00a0causes\u00a0acute pain.<\/p>\n

This Sickle Cell Life<\/a><\/em>\u00a0is part of\u00a0the\u00a0work of\u00a0DEPTH<\/a>\u00a0research group\u00a0at\u00a0LSHTM<\/a>.\u00a0The project\u00a0explores how people move from using child to adult healthcare services and\u00a0asks\u00a0young people\u00a0about their\u00a0experiences of living with sickle cell.\u00a0Transitioning to adulthood is obviously not just\u00a0something that affects clinical experiences and so we\u00a0also\u00a0explore\u00a0education and relationships,\u00a0and ask young people what is important to them.<\/p>\n

At the BSA\u00a0MedSoc\u00a0conference we talked about how healthcare transitions shape the identity of young people, and how these transitions\u00a0help \u2018make\u2019\u00a0particular kinds\u00a0of patients.<\/p>\n

Why focus on identity? Health transitions\u00a0need\u00a0new\u00a0health knowledge and\u00a0new\u00a0behaviours\u00a0to\u00a0develop, but\u00a0they also need\u00a0development of\u00a0self-perceptions\u00a0and\u00a0understandings of how\u00a0a person\u00a0should behave as\u00a0an\u00a0adult\u00a0(rather than a child)\u00a0patient.\u00a0In this way, identities play an important\u00a0role in shaping health practices\u00a0and beliefs.\u00a0Understanding identity development during transitions can\u00a0help\u00a0explain\u00a0why some young people\u00a0transition\u00a0smoothly or less smoothly\u00a0into healthy\u00a0adulthood.<\/p>\n

In our presentation,\u00a0we talked about how\u00a0ideas\u00a0and discourses\u00a0about\u00a0self-management and healthy lifestyle\u00a0within\u00a0healthcare today\u00a0act\u00a0as a way to\u00a0discipline young people\u00a0\u201cat a distance\u201d, to quote\u00a0Miller & Rose (1990)<\/a>\u00a0and\u00a0influence\u00a0their behaviour. These healthcare self-management\u00a0discourses\u00a0intersect with demands from\u00a0schools\u00a0that are often\u00a0not compatible.\u00a0For instance, schools\u00a0demand that\u00a0young people excel and become entrepreneurial, competent\u00a0individuals, but excelling at school is far more difficult for young people who\u00a0have to\u00a0spend time in hospital, or who have to rest regularly to avoid having a pain crisis.\u00a0These intersecting demands can\u00a0translate into conflicting\u00a0\u201cself-disciplining\u201d\u00a0identities.<\/p>\n

For\u00a0young people with sickle cell, we found that\u00a0transitions to adulthood involve\u00a0relentless self-disciplining and self-surveillance to\u00a0try to be as healthy as possible,\u00a0while\u00a0also\u00a0aspiring to work\u00a0hard so that they can\u00a0develop and\u00a0meet\u00a0educational and career\u00a0goals.<\/p>\n
