Dans un entretien au Guardian d\u00e9but avril 2014 <a title=\"\" href=\"#_edn1\">[i]<\/a>, le pr\u00e9sident de la Banque Mondiale surprit tout le monde en affirmant que son institution avait promu pendant des ann\u00e9es le paiement direct au point de service sur la base d\u2019une id\u00e9ologie. On se rappellera que cette m\u00eame organisation publiait en 1985 un rapport affirmant que \u201c<i>faire payer les usagers dans les centres de sant\u00e9 publics faciliterait l\u2019acc\u00e8s des pauvres<\/i>.\u201d <a title=\"\" href=\"#_edn2\">[ii]<\/a>. En 2014, l\u2019auteur principal revenait lui aussi sur ses propos en affirmant que le paiement direct \u00e9tait \u00ab\u00a0<i>une pratique qui en r\u00e9alit\u00e9 impos[ait] un fardeau sur les malades et les personnes qui ont des besoins, un segment petit et vuln\u00e9rable de la population <\/i>\u00bb <a title=\"\" href=\"#_edn3\">[iii]<\/a>. Aujourd\u2019hui, il semble donc se d\u00e9gager un consensus international contre le paiement direct au point de service et en faveur de leur abolition <a title=\"\" href=\"#_edn4\">[iv]<\/a>. Les agences des Nations Unies, l\u2019union europ\u00e9enne (2009) et l\u2019Union Africaine (2010) l\u2019avaient r\u00e9clam\u00e9e. La Banque Mondiale semble donc emboiter d\u00e9finitivement le pas.<\/p>\n<p>L\u2019\u00e9volution de ces id\u00e9es <a title=\"\" href=\"#_edn5\">[v]<\/a>, si elles ne sont pas encore totalement partag\u00e9es par tous les acteurs de terrain <a title=\"\" href=\"#_edn6\">[vi]<\/a>, s\u2019explique en grande partie (mais pas uniquement) par l\u2019accumulation des preuves scientifiques. Ainsi, la preuve du caract\u00e8re injuste et non n\u00e9cessaire du paiement direct, dont plusieurs articles de HPP ont fait \u00e9cho (voir la synth\u00e8se de 1997 <a title=\"\" href=\"#_edn7\">[vii]<\/a>), a \u00e9t\u00e9 faite. Depuis maintenant 10 ans, s\u2019est aussi accumul\u00e9e une s\u00e9rie de donn\u00e9es probantes sur les effets de l\u2019abolition des paiements directs sur le recours aux services. Plusieurs synth\u00e8ses ont \u00e9t\u00e9 publi\u00e9es dans HPP et ailleurs <a title=\"\" href=\"#_edn8\">[viii]<\/a> <a title=\"\" href=\"#_edn9\">[ix]<\/a>. L\u2019article de McKinnon B. et al (2014) publi\u00e9 dans ce num\u00e9ro de HPP s\u2019inscrit dans ce processus d\u2019accumulation de preuves <a href=\"http:\/\/heapol.oxfordjournals.org\/content\/early\/2014\/05\/10\/heapol.czu027.abstract\">[x]<\/a>. Cette recherche est particuli\u00e8rement int\u00e9ressante car elle est une des rares \u00e0 utiliser des donn\u00e9es populationnelles. Elles confirment les effets b\u00e9n\u00e9fiques de l\u2019abolition des paiements directs pour le recours aux soins et la r\u00e9duction de la mortalit\u00e9 n\u00e9onatale, rejoignant les conclusions d\u2019autres \u00e9tudes au Niger <a title=\"\" href=\"#_edn10\">[xi]<\/a>, au Burkina Faso <a title=\"\" href=\"#_edn11\">[xii]<\/a> et en Tha\u00eflande <a title=\"\" href=\"#_edn12\">[xiii]<\/a> sur d\u2019autres indicateurs de sant\u00e9.<\/p>\n<p>Malgr\u00e9 ces enseignements, cette \u00e9tude nne permet pas de saisir les enjeux de la complexit\u00e9 d\u2019une telle intervention, pourtant immenses <a title=\"\" href=\"#_edn13\">[xiv]<\/a>. Il ne s\u2019agissait certes pas de l\u2019objectif des auteurs. Pourtant, cela devrait devenir une priorit\u00e9 de la communaut\u00e9 scientifique. Alors que les connaissances quantitatives sur le recours aux services de sant\u00e9 s\u2019accumulent, les recherches interdisciplinaires usant de m\u00e9thodes mixtes, essentielles pour comprendre ces interventions complexes dans leur contexte <a title=\"\" href=\"#_edn14\">[xv]<\/a>, en situation r\u00e9elle, restent trop rares. Notre derni\u00e8re recension sur le sujet montrait qu\u2019une approche syst\u00e9mique manquait cruellement <a title=\"\" href=\"#_edn15\">[xvi]<\/a>. Et cela n\u2019est pas impossible \u00e0 r\u00e9aliser, comme le montrent les travaux d\u2019\u00e9quipes de chercheurs au Burkina Faso, Mali, Niger, Zambie et d\u2019autres qui devraient bient\u00f4t \u00eatre partag\u00e9s sur le B\u00e9nin et le Maroc.<\/p>\n<p>La d\u00e9monstration de l\u2019efficacit\u00e9 de l\u2019abolition du paiement direct ayant \u00e9t\u00e9 faite, il faut d\u00e9sormais soutenir les d\u00e9cideurs en leur apportant des connaissances sur les d\u00e9marches appropri\u00e9es pour la financer, la mettre en \u0153uvre, en am\u00e9liorer l\u2019\u00e9quit\u00e9, l\u2019adapter au contexte et aux syst\u00e8mes de sant\u00e9, comprendre le r\u00f4le des acteurs sociaux, etc. Certains travaux sur ces sujets commencent \u00e0 voir le jour <a title=\"\" href=\"#_edn16\">[xvii]<\/a>, mais ils restent en nombre insuffisants. Les chercheurs et les organisations qui financent la recherche ne s\u2019int\u00e9ressent-ils pas \u00e0 ces enjeux ? Comment expliquer par exemple que l\u2019Alliance for Health Policy and Systems Research vient de lancer un appel pour mieux comprendre la mise en oeuvre de la strat\u00e9gie de financement bas\u00e9 sur les r\u00e9sultats, ce qui est tr\u00e8s pertinent, alors que cela n\u2019a jamais \u00e9t\u00e9 r\u00e9alis\u00e9 pour les interventions d\u2019exemption du paiement direct\u00a0? Choix scientifique ou id\u00e9ologique\u00a0?<\/p>\n<p>La production de donn\u00e9es probantes sur la complexit\u00e9 des interventions d\u2019exemption du paiement des soins est indispensable pour soutenir la mise en \u0153uvre de la couverture universelle r\u00e9clam\u00e9e en mars 2014 par l\u2019OMS et l\u2019Union Africaine <a title=\"\" href=\"#_edn17\">[xviii]<\/a> et attendue pour 2030 par tous les acteurs internationaux <a title=\"\" href=\"#_edn18\">[xvix]<\/a>. Il serait donc salutaire, et n\u00e9cessaire, qu\u2019une nouvelle g\u00e9n\u00e9ration de recherches centr\u00e9es sur l\u2019individu et pr\u00e9occup\u00e9es par la complexit\u00e9 de ces interventions se d\u00e9ploie. Appel aux jeunes chercheurs, voix \u00e9mergentes <a title=\"\" href=\"#_edn19\">[xx]<\/a> et autres scientifiques qui s\u2019inscrivent dans l\u2019approche centr\u00e9e sur l\u2019individu !
